By checking this box I am agreeing to the following:

1. I certify that by registering, I am a doctor with a post-graduation degree and/or diploma in Otorhinolaryngology or Plastic Surgery. I certify that I chose the registration type that reflects my specialty. If that statement is not true, I forfeit any fees paid and relinquish my right to attend the course.
2. All RSI and PAAFPRS Members must upload documentation of membership to confirm their registration. Failure to do so will result in a penalty.
3. I certify that I understand my contact information will not be sold or shared with any outside party. Participating exhibitors will have access to my contact details, solely for marketing and educational purposes.
4. I hereby grant RIAMS the permission to use my likeness in a photograph in any and all if its printed and digital publication platforms.
5. Cancellations made within 8 weeks prior to the course are subject to a $500 administrative fee. Cancellations received after this period will not be eligible for a refund. cancellations must be submitted in writing to info@riams.co.in Refunds will be process after 15 days of post conference. No-shows will not be eligible for refunds.
